This process will allow you to submit a waiver for your participant or participation in public events at the Colorado Sports Center.
A waiver is required by all adult (over 18 years) participants or needs to be reviewed and signed by a parent for participants under the age of 18 years old.
A waiver for a participant is required for the following Colorado Sports Center programs:
